Hospice is a special kind of caring that helps you live with dignity, comfort and peace at the end of life. One of our greatest fears is dying alone, away from those we love and away from home. If you would like to be in a caring environment, surrounded by those you love, then hospice may be an option for you.

What does Hospice do?

  • Supports you and your family as you live at the end of life.
  • Provides care in your home under the direction of a medical team.
  • Helps keep pain and other distressing symptoms managed.
  • Provides emotional support to your family before and after your death.

How do I qualify for Hospice care?

  • Your primary care physician has indicated you have an illness with a life expectancy of months rather than years.
  • You are ready for comfort or palliative care and want no further invasive treatments.
  • You have a primary caregiver available to you, are willing to discuss caregiving options, or are a resident of a long term care facility.

What services are provided by Hospice?

  • Medical care coordination with your physician.
  • Nursing care including personal care if needed.
  • Medication, medical supplies and equipment.
  • Counseling for you and your family members.
  • Volunteer services.
  • Bereavement services.

  • Talk with your discharge planner or primary care physician.

Who pays for Hospice care?

  • Medicare and Medicaid offer comprehensive coverage under a special benefit.
  • Private insurance may cover hospice care - Hospice staff can check with your provider.
  • Occasionally, a family will choose to pay for hospice care.
